Telehealth Consent
Effective September 2026
This Telehealth Consent explains the nature of care delivered through telehealth on the West Labs Rx platform. You will be asked to acknowledge this consent before your first evaluation with a provider.
1. What Telehealth Is
Telehealth is the delivery of healthcare services using electronic communications — such as secure messaging, questionnaires, and video visits — when the patient and provider are not in the same physical location. Through West Labs Rx, independent licensed providers evaluate your intake information and, where appropriate, diagnose and prescribe treatment remotely.
2. Potential Benefits
Telehealth can offer more convenient access to care, reduced travel and waiting room time, and timely follow-up with your care team.
3. Potential Risks and Limitations
Telehealth has limitations. Providers cannot perform a physical examination, and information transmitted electronically may be incomplete or delayed. Technical failures can interrupt a visit. In some cases, the information obtained through telehealth may be insufficient for a provider to make a diagnosis or treatment decision, and you may be referred for in-person care.
Telehealth is not appropriate for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.
4. Your Acknowledgements
By consenting to telehealth, you acknowledge that: you have the right to withhold or withdraw this consent at any time without affecting your right to future care; the laws that protect the confidentiality of your medical information also apply to telehealth; you are responsible for providing accurate and complete health information; and a prescription is issued only when the provider determines it is medically appropriate — completing an evaluation does not guarantee a prescription.
5. State Availability
Telehealth services and specific treatments are available only in states where the treating provider is licensed and the treatment is permitted. Availability varies by state and may change over time.
